Novel Psychoactive Substances (NPS) are a diverse group of synthetic substances created to mimic the effects of scheduled or illicit drugs; however, they may vary widely in both toxicity and potency from the drugs they are intended to mimic. Synthetic stimulants, along with synthetic cannabinoids, are NPS for which traditional toxicology testing is widely available. However, it is important to know what compounds are in circulation in the illicit drug supply when evaluating a test menu, as a single synthetic stimulant’s availability typically rises and falls over time in response to regulation and is replaced with new compounds. Screening for drug abuse and illicit drugs in human urine, saliva, and other body fluids is a key application in forensic medicine and toxicology labs, de-addiction centers, DOA and NPS detection labs, narcotics detection laboratories, and clinical and forensic toxicology laboratories. Rapid and unequivocal detection and identification of drugs and related compounds in numerous samples are essential requirements for drug screening. On a positive 2023 note, in May Maryland Governor Wes Moore signed into law The Josh Siems Act, the first bill of its kind, and one that requires emergency rooms to include fentanyl testing in toxicology screens.After the fatal overdose of a 31 year old who passed from fentanyl poisoning but wasn't tested for it when admitted to the ER, pressure was put on lawmakers to put this life saving measure into place. Other cities and states are starting to follow. During standard toxicology screenings, many hospitals run what is called the “Federal 5,” which screens for drugs, such as cocaine and cannabis. Fentanyl is not always included in that test unless requested, and the panels are not very accurate in detecting the substance. Bill SummaryAN ACT concerning Hospitals - Overdoses - Testing for Fentanyl (The Josh Siems Act) FOR the purpose of requiring the emergency department of a hospital to conduct a rapid include testing for fentanyl as part of a urine drug test for fentanyl in patients with a known or suspected drug overdose if a toxicology report is requested by a treating health care provider screening; and generally relating to fentanyl testing for hospital patients with a known or suspected overdose.
